Here is a better copy of this booklet. The original is 9-1/2 X 12 inches, which is one of the reasons people have had problems scanning it. I bought an original and scanned it at high resolution (600 dpi) on an 11 X 17 inch scanner.

I did not behead MG Connolly on p 12. That was done in the original printing, not the scanning.
